About Us:  A nonprofit mental health agency with multiple sites in Alameda County and an operating budget of $14.5 million. Founded in 1971, BHI provides social rehabilitation including a full range of Medi-Cal services to adults with co-occurring serious mental health and substance use disorders.


Position Summary
The Case Manager will join our dedicated team in a licensed Transitional Residential Treatment program. This role will involve providing comprehensive mental health support services to individuals with behavioral health needs. The incumbent will play a crucial role in facilitating rehabilitation, recovery, and community integration for our clients.

Primary Responsibilities

  • Conduct comprehensive assessments of clients’ mental health needs, strengths, and goals
  • Develop individualized, person-centered rehabilitation plans using evidence-based practices
  • Provide individual and group counseling focused on mental health, coping skills, and life skills development
  • Implement behavioral interventions and therapeutic techniques to support recovery goals
  • Monitor and evaluate client progress, adjusting treatment plans as needed
  • Coordinate care with interdisciplinary teams including psychiatrists, nurses, case managers, and community partners
  • Facilitate psychoeducational groups (e.g., stress management, conflict resolution, relapse prevention)
  • Advocate for client access to resources such as housing, employment, and healthcare
  • Maintain accurate and timely documentation in compliance with regulatory standards
  • Participate in staff meetings, case conferences, and ongoing trainings
  • Ensure program safety and follow established overnight and emergency procedures
  • Perform additional duties as assigned

Qualifications & Requirements

  • Bachelor’s degree in Psychology, Social Work, Counseling, or related field (preferred).  Active registration with BBS required.
  • Minimum 2 years of experience in mental health or behavioral healthcare settings
  • Experience working with individuals with severe mental illness preferred
  • Ability to obtain criminal background clearance (per CA WIC § 5405 and agency requirements)
  • Must pass post-offer physical and TB clearance (per state licensing requirements)
  • Valid driver’s license, reliable vehicle, and auto insurance required
  • Clean driving record (MVR) to meet agency insurance standards
  • Ability to travel locally as needed

Pay rate: $28.85 – $33.65 hourly  DOE
