Job Description

Schedule: Full-Time

Duties:

  1. Facilitates care under the direction of the Primary Therapist and Clinical team.
  2. Handles case coordination by communicating with relevant parties, scheduling and gathering information.
  3. Determines clients’ requirements by completing intake interviews; determining need for therapeutic medical, psycho-social, and psychiatric evaluations; reviewing therapist evaluations, treatment objectives, and plans.
  4. Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
  5. Monitors cases by verifying clients’ attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
  6. Maintains clients’ records by reviewing case notes; logging events and progress, meets with clients weekly 1:1 to assess progress and provide support.
  7. Communicates clients’ progress by conducting weekly interdisciplinary meetings and evaluations, disseminating results and obstacles to therapeutic team and family, as well as identifying treatment influences.
  8. Works closely with the Director of Client Relations and Clinical team to prepare clients’ discharge by reviewing and amplifying discharge plans, orienting and training family members.
  9. Coordinates discharge and post-discharge requirements, such as future referrals or placements.
  10. Continual follow up with clients after discharge at regular intervals to assess progress and program compliance.

Requirements:

  1. 1+ years experience working in mental health, PHP or residential settings.
  2. 1 year experience as a Case Manager preferred.
  3. Experience with discharge planning preferred.
  4. Strong communication skills including both written and verbal.
  5. Strong computer skills including Microsoft Suite and the ability to create reports in Excel.
  6. Current and valid California Driver’s License.

Compensation:
Bachelor’s Degree: $25-$28 per hour, plus benefits and room for growth.
Master’s Degree: $27-$30 per hour plus benefits and room for growth.
